第一步:高中升入大学的选择

1. 藤校(名校)vs 大学医学院连读
首先我们是在确定孩子基本决定学医的情况下讨论这个问题。如果孩子还没有准备好,或者犹豫,这个选择就不存在了。
大学和医学院连读的program在美国不多,名校只有几所(e.g. John Hopkins, Wash U StL, Northwestern, and Brown),当然竞争相当激烈,非常难进. 如果能够进入这些combinded program的学生,一般是可以进入HYPS等大藤一类的顶尖大学,那么这样的学生即使不选择combined program, 他们将来要进医学院也一定不是问题。况且也给他们接触别的专业开拓新的视野的机会。四年以后,如果他们还坚持学医,那是真正的心有所属,一定错不了。还有对于家长的经济投入,两种选择没有区别。
美国大学和医学院连读的program比较多的是在州立大学(不是每个州都有的, e.g. New Jersey 有,Massachusetts没有)和一些不是最顶尖的私立医学院 (e.g. University of Miami),竞争也是非常激烈,它们的目的是吸引优秀的学生;如果是州立的,一般都是首先招收本州学生。我比较熟悉的当然是我们所在的州立康州大学(University of Connecticut),每年只有10个左右的名额。进入这个program的学生,也都是可以进入藤校/名校的高中学生。我认为,如果选择combined progra,你的孩子可能会失去比较良好的大学教育。这个纠结是一直困扰着我的,完全是个人的想法和儿子的实际情况分析。儿子个性阳光随和,但是不太努力,他说他愿意学医,我们做父母也很希望他学医,所以就报了这么一个combined program试一下。没想到被录取了,同时也被几个藤校/名校录取(他没有被HYPS录取),其中他最喜欢的是Dartmouth。 他在Dartmouth和uconn BS/MD 之间犹豫了很久,最后决定去uconn BS/MD主要原因是想将来有保障,大学可以玩儿了。我先生是坚决支持儿子去uconn BS/MD,因为他非常希望儿子学医,不想让他有太多机会explore, 这时另外一种家长隐性引导式,利弊这里不做讨论;我是一直不支持,因为我对于巨大的公立学校的教育一直持有保留看法,几百人一堂课,如何从睿智的教授那儿学到什么;我认为孩子在大学不仅是学知识,更主要是学习理念,学习一种如何学习的能力,学习对于未来的预测和如何适应变化的能力,在这种想法下,我当然希望儿子去他喜欢的Dartmouth,受到最好的大学教育。在我们和他分析了所有的利弊后,他也和一些uconn医学院的同学聊了以后,他决定走捷径去这个combined program,那我们也是支持了。这种州立的combined program,家长还有投资上的优势,因为基本上大学部分是全奖学金(merit based scholarship),以后只要付州内医学院的学费。

2. 怎么在高中准备孩子申请大学医学院连读:
我们也没有特别准备在高中期间申请这种program,我儿子是我们朋友圈里第一个申请进入这个program的,所以我们也没有经验。高中学业当然和各位孩子一样,在学校修最难的课程,一共11门AP,数理化生物都有;体育是游泳和crew; leadership 是class senate;打工是教小孩游泳和在中文学校教数学和美术。高中期间也没有得过什么特殊的奖,除了油画得过local的一等奖,和national merit scholarship 。
但是我们在安排暑假活动时,是准备把他往学医的方向努力的。??
Sophomore结束的暑假他去了Youth Leadership Forum summer camp – Youth Leadership in medicine (在Boston),让他explore一下是否喜欢学医,他回来后比较受鼓舞,说可以学医,在Camp, 接触了Harvard med school students, 和一些实习医生, 记得他说:“Smart people are cool too”。Junior暑假我通过朋友在西北大学医学院给儿子找了一个做intern的机会,他一个人在芝加哥住在我朋友家,在那儿做了五周有关”冰冻人“的科研,那个MD教授是”冰冻人”领域的学科先驱,这样儿子后来写申请essay为什么要学医时,也有点真实的感情,有血有肉;教授挺喜欢他,还给他写了进combined program的推荐信。

3. 申请选校:
Combined program 有4+4 (8年),3+4 (7年)。以前有6年的,现在好像没有了,因为作为医生,还是需要一定的成熟度,也需要人文方面的培养。
Combined program:有的是真正的连读,连大学学位都没有,这样学生就必须在这个program一直走下去了;有些program,有BS/BA学位,但是你如果要再申请别的医学院,你就要放弃这个直升名额,比如Brown就是这样的policy; 那最好的就是那些给BS/BA学位,可以再去申请别的医学院,而这个直升名额给你保留,我儿子的uconn就是这样的policy。
说是大学毕业后直升,但是每个program也都有一定的进入医学院的最低要求,比如我儿子的program, 直升的条件是大学GPA 3.67, MCAT 30就可以直升。这些要求一般都是很容易达到的;也有一些很难的条件,比如wash U StL的直升条件就难。所以高中生在申请时这些条件都要了解清楚.

4. 面试:
申请Combined program一般都是在申请大学的材料基础上,再有附加的申请材料。主要是要知道你为什么要学医。
申请以后,就会通知学生去medical school面试(我们康州大学是7-10%的申请者去面试),这个面试和medical school的面试很像,也是几个学生分组集体面试然后个体面试,面试是由medical school的教授 医生和医学院学生组成的阵容, 面试时间好像是2-3个小时左右,家长不能进去的。我们当时是朋友圈里的第一个,也没有先例,儿子也不怎么感兴趣,所以我们一点都不紧张,没有准备,也不知道怎么准备,儿子连西装都不原意穿(当然是穿了business casual),结果可能是歪打正招,就被录取了(录取率是3%左右)。

第二步:大学学习

我这个主要是结合我儿子的uconn的情况来谈体会和经验教训。
进了这个program后,很多州立公立大学的缺点就显露出来,最大的问题是学生的advisor, 什么也不懂,也不管,反正对于儿子的选课,将来去medical school要做些什么准备什么一概不管;我们家长还觉得这些program的学生是否会有一些别的特殊课程,结果好像也没人管。

uconn这样的公立大学,任何AP成绩都可以转换,结果儿子11门AP统统转入大学学分后,他就不用修任何人文方面的课了(2门AP English,两门AP history),这下正好合了孩子的心意; 他想学油画课,不可以,说是他们的绘画课只给艺术学院的学生修,因为师资有限。这些都是我担心的,也都发生了,好在儿子性格非常随和,想学的不行,就学别的,什么都无所谓。有了这么多AP课,他可以提前毕业`,也可以修几个major, 但是不能提前去medical school, 因为名额是lock住的,学生可以gap一年,再回来上med school. 儿子要享受完整大学四年,就有了两个major,两个minor (什么major都可以上医学院,只要完成医学院入学的课程要求,每个医学院网上都有,也是大同小异).
因为这些AP,没有在大学里重修的课程给他后来申请别的医学院和考MCAT带来一些不利。到了要考MCAT才发现, 物理和英语是三个section中的两个,我们都傻眼了,这就是那些大学里advisor都不管的事;进了大学后,我们家长也很少管他的事了,幸亏经过几周的复习,把高中AP物理拣了回来,物理这个section考得相当不错(14/15),但是英语部分就考得不太理想了(10/15)。

到了申请医学院时,又发现除了常规知道的几门生物化学课外,物理和英语都是入学的必修课程,有些医学院根本不承认AP,有些医学院强烈建议不要用AP,建议在大学重修。亡羊补牢,儿子在大四补修了一个学期的物理和一个学期的英语写作,可是人家都是要求一年的课程。

进了这个program的好处是孩子可以非常enjoy大学的生活,所以儿子毕业的时候说:“This is the best time in my life!” 他在uconn学习挺轻松的,GPA 有3.9 以上;参加了兄弟会,一直努力非常投入,做了很多也组织倡导了很多volunteer/fund raising的events,一直竞选争取当领到,这个理想也实现了。

在大学期间暑假安排:第一个暑假在Pfizer summer intern, 做和药物安全有关的课题; 第二个暑假在uconnmedical school summer intern, 跟着Doctor shadowing, 是这个program 的学生都可以申请,他们都直接进手术室和门诊;第三个暑假一边在hospital 跟着Doctor shadowing, 一边又到Pfizer summer intern, 做药物代谢方面的课题。
MCAT是在 大三的冬天考的,申请是在大三结束的暑假完成的。  

第三步:医学院申请

一般的医学院申请步骤和要求就不在这儿讨论了,这里主要谈谈从combined program 申请医学院的特点,纯属个人经验,并不放之四海而皆准。
好处:因为有了保底,又是州立大学,享受州内学费,所以是一个不错的选择,申请的时候只要选自己的理想学校就行了,不用申请20-30个学校;何况就做医生,不做research, 可能医学院的排名没有像法学院和商学院那么重要。

申请到别的医学院是儿子自己的理想,这个一直贪玩的孩子,突然非常努力,要认真考MCAT,把申请学校的事搞得一清二楚。因为儿子喜欢大城市,也可能受我的“名校情节”的耳濡目染,就是想离开uconn,去大城市,去更好的学校,所以他申请了8所医学院,分布在波士顿 芝加哥 迈阿密 洛杉矶 (不喜欢纽约)。我们以为已经是combined program 里的学生,申请的时候会有优势,成绩也不错,profile也可以,也就没怎么管他的学校选择了。

缺点(也只是逻辑推测的):其实是combined program 里的学生,申请的时候没有优势,因为和uconn相近名次的医学院都不给他interview的机会,比如Tufts(儿子喜欢波士顿城市才申请的);而university of Miami,虽然给了他interview的机会,但给了他waitlist(儿子喜欢热带地区才申请的),现在我们才明白医学院有个“yield protection”,他们不想录取以后的升入率低(enrollment rate). 他们不想浪费名额,有了州立的医学院,为什么还要考别的呢,特别是排名相近的付几乎多一倍的学费。当然像Harvard and Stanford这种顶尖医学院,可能觉得uconn的大学training不够格,也没有interview。 我朋友的小孩upenn的,MCAT没有特别高,GPA是3.9以上,当然很优秀,进了Harvard Med school。由此推测,名校进名医学院还是有一定的道理的。当然,儿子幸运地进了他非常喜欢的位于芝加哥市中心的西北医学院, 我们也很高兴。

第四步:医学院就读 - 未来篇


希望儿子能够吃得起苦,能够真正热爱医学工作。一切留着以后分享,因为今年8月才入学…

也祝愿所有的孩子都心想事成,发现自己的爱好,读喜欢的书,做喜欢的工作......
